INSURANCE AND FEES

Our cash pay/out of network rates for counseling and mental health services are $185 for the initial consultation and $175 for each follow up session.

 

Dr. Strait is considered an "out-of-network" provider.  She is able to provide you with a receipt for insurance reimbursement (also known as a "superbill") to provide to your insurance company.

 

Dr. Mundy is in network with Blue Cross/Blue Shield, Aetna and Cigna.

 

All other insurance companies are considered "out of network."  Prior to your initial consultation, please provide us with your insurance information.  We will file insurance claims for in-network providers only.  

 

For "out of network" coverage, we will be happy to provide you with a statement of insurance reimbursement to provide to your insurance company.  You may seek reimbursement directly from your insurance company if your insurance provider covers a portion of your out of network care.   Please contact your insurance company to verify your benefits before scheduling.
